Provider ID: For most users this will be the Medicare Provider ID (CMS Certification Number or CCN) for your hospital. This is a six-digit number with the form "NNNNNN."

Username: Your designated username, as assigned by your LTRAX™ facility or organization administrator(s).

Password: Your designated password, as assigned by your LTRAX facility administrator and/or updated by you. Your password must be at least eight characters long. It must include uppercase and lowercase letters, at least two numbers, and at least one special character (~ ! @ % ^ & * _ - + = ` | ( ) { } [ ] : ; " ' < > , . ?). It cannot include any part of your username, your facility’s name, or the word "password." Your LTRAX facility or organization administrator(s) establishes and can reset your password.

NOTE: If you are unable to log in, make sure your Medicare Provider ID is in the Provider ID box and click Forgot your password? to display a list of your LTRAX facility administrators, any of whom can reset your password and help you log in.
